Cheryl R. Blanchard, Ph.D.

Director

Dr. Blanchard was most recently Senior Vice President and Chief Scientific Officer of Zimmer Holdings, Inc. Dr. Blanchard was also General Manager of the Zimmer Biologics business, a venture business within Zimmer that she built through a combination of internal R&D; efforts and technology licensing/acquisition activities. Previously, she was Senior Vice President of Research and Development, Vice President of Corporate Research and Clinical Affairs and Vice President of Research and Biologics, also with Zimmer Holdings, Inc. Prior to joining Zimmer in October of 2000, Dr. Blanchard served in leadership roles at the Southwest Research Institute, the University of Texas Health Science Center and the Oak Ridge National Laboratory. Dr. Blanchard also served as a Director of ISTO Technologies, Inc and was the founding board chair of OrthoWorx. She holds a Bachelor of Science degree in Ceramic Engineering from Alfred University, Alfred, New York, and a Masters of Science and Ph.D. in Materials Science and Engineering, both from the University of Texas at Austin.

Debora LaBudde

Director

Debora is a founder of Envoi Ventures, a venture capital fund which makes investments in seed and early stage life science and technology/IT companies. The firm co-invests with other leading venture firms and its principals often take an active role with companies prior to and following an investment. Earlier in her career, Ms. LaBudde was Director of Business Development for Bausch & Lomb, Inc. where she was involved in over a dozen successful M&A; and licensing transactions representing more than $1.5 billion of aggregate value for the company. Ms. LaBudde is also an advisor and served as interim CEO of a Calorics Pharmaceuticals focused on inflammatory diseases assisting the company with licensing university technology, building the company strategy and raising a seed funding round with top tier investors in December 2010. She received an MBA and BA in Economics, Magna Cum Laude, from the William E. Simon School of Business at the University of Rochester.

Donald W. Grimm

Director

Mr. Grimm retired from Eli Lilly & Company, a research-based pharmaceutical company, in December 1993 after 23 years of service. Mr. Grimm held various executive positions at Lilly. Following these assignments, Mr. Grimm was President and CEO of Hybritech, Inc., a wholly owned subsidiary of Lilly. Since June 1995 he has served as Chairman and President of Strategic Design, LLC, a strategic planning and consulting company. Mr. Grimm has been a director of Hamilton BioVentures, LLC, a $100 million fund focused primarily on life sciences, since August 2001. In addition, he is currently a director of several private life science companies. Mr. Grimm has been on the board of Life Technologies for approximately 12 years. During that time period Life Technologies grew from $30 million in revenues company to their current annual revenue of over $3 billion. Mr. Grimm received his B.S. in pharmacy and his M.B.A. from the University of Pittsburgh.

Richard L. Intrater

Director

Mr. Intrater has been in the investment banking and financial services industry for over 30 years. He is the President & CEO of Long-Term Solutions, a Multi-Family Office he founded in 2005 to assist multi-generational families of significant net worth with financial, investment, philanthropic and administrative oversight. He also oversees LTS Capital Partner I and II which provides private equity investment capital to companies primarily in the clean technology field. His previous experience includes a role as board member and managing director of Sand Hill Advisors and the founder of Sutter Street Capital Management. Mr. Intrater spent 13 years as Senior Vice President of Lehman Brothers in New York and San Francisco and began his career as a financial analyst for the U.S. Export Import Bank in Washington, D.C. Mr. Intrater is the Founding and past Chairman of the Board of Directors, KIPP Bay Area Schools, past President of the Boys and Girls Clubs of San Francisco, former National Trustee of the Boys and Girls Clubs of America, former Board Member of the Coyote Point Museum of Environmental Education, Former Board Member of the Family Service Agency of San Mateo County, and an observer to the board of Fallbrook Technologies. Mr. Intrater graduated with a BA in finance at the University of Arizona and continued graduate work in International Studies at the University of Missouri.

Gary E. Jacobs

Director

Mr. Jacobs is the Managing Director of Jacobs Investment Company, which participates in real estate development and other investment activities throughout the United States and abroad. In addition, he has multi-million dollar investments in several other venture capital funds. He also owns and operates a professional minor league baseball team, the Lake Elsinore Storm, affiliated with the San Diego Padres. Mr. Jacobs serves as Chairman of the Board of Trustees for High Tech High, a public charter high school, Vice-Chair of the Jewish Community Center Association Continental Board and is a board member of the Lawrence Family JCC and the UCSD Board of Overseers. Mr. Jacobs's other philanthropic work includes being a past president of the United Jewish Federation of San Diego County. Additionally, Mr. Jacobs and his wife created and funded the Gary and Jerri-Ann Jacobs International Teen Leadership Institute which promotes Muslim / Jewish understanding. Prior to his current investment and philanthropic activities, Mr. Jacobs worked as a software engineer and senior education specialist at QUALCOMM, Inc. Mr. Jacobs holds a B.A. in Management Science from the University of California at San Diego.

Douglas W. Kohrs

Director

Mr. Kohrs has over 30 years of experience in the medical device industry. He was most recently President and CEO of Tornier N.V. a publicly traded orthopedics company and was previously President and Chief Executive Officer of American Medical Systems Holdings, Inc., a publicly held medical device company (acquired by Endo Pharmaceuticals Holdings Inc. in 2011) and served as Chairman of the Board of the company until May 2006. Prior to joining American Medical Systems Holdings, Inc., Mr. Kohrs was General Manager of Sulzer Spine-Tech Inc., an orthopaedic implant manufacturer of which he was a founding member beginning in August 1991. During the past 10 years, Mr. Kohrs has served on the board of directors of nine different medical device companies, including ev3 Inc., a publicly held medical device company (acquired by Covidien Group S.a.r.l in 2010), and Kyphon, Inc., a publicly held medical device company that (acquired by Medtronic, Inc. in 2007). Mr. Kohrs holds a Master of Business Administration from Northeastern University, a Bachelor of Science in Bioengineering from Texas A&M; University and a Bachelor of Arts in Engineering Sciences from Austin College.

Richard Mott

Mr. Mott is currently a principal in Walkabout Consulting LLC, a management consulting and private equity firm. Mr. Mott most recently served as President and Chief Executive Officer and Director of Kyphon Inc., a NASDAQ-listed global medical device company focused on minimally invasive spinal therapies, through its acquisition by Medtronic, Inc in late 2007. Prior to Kyphon, Mr. Mott held several executive positions at Wilson Greatbatch Technologies, Inc., most recently as Chief Operating Officer, as well as executive operating roles within the Bristol-Myers Squibb Company. Mr. Mott is the previous recipient of the Ernst & Young Entrepreneur of the Year Award and the Frost & Sullivan Technology Leadership of the Year Award. Mr. Mott holds a B.S. in Ceramic Engineering from Alfred University and is a Graduate of Harvard University's Advanced Management Program.

Steven R. Garfin MD.

Dr. Garfin is Professor and Chairman of the Department of Orthopaedics at the University of California. Additionally, Dr. Garfin holds hospital positions at the University of California, San Diego Medical Center, and the La Jolla Veterans Administration Hospital. Dr. Garfin is a Board Certified Spine Surgeon who is a past president of the North American Spine Society (NASS) and Cervical Spine Research Society (CSRS) and is one of the most respected spine surgeons in the United States. Dr. Garfin's impressive published research and writing include: author of nine books, 123 book chapters, 255 peer-reviewed journal articles, and 132 abstracts.

Scientific Advisory Board

Thomas Bauer MD, Ph.D.

Dr. Bauer specializes in orthopaedic and gastrointestinal pathology. He holds joint appointments in the Institutes of Orthopaedics and Rheumatology as well as Spine, and his research involves mostly orthopaedic biomaterials. He is considered an expert in the mechanisms of success and failure of orthopaedic devices and he has published more than 225 manuscripts and book chapters. He is also Deputy Editor for Research for The Journal of Bone and Joint Surgery.

Gabi Niederauer, Ph.D.

Dr. Niederauer has more than 20 years of experience in the study, development, and commercialization of biomaterials for orthopedic, dental, and ENT tissue repair. She is currently the VP of R&D; at ENTrigue. Previously she served as Director of R&D; at Osteobiologics, Inc. She is experienced in the coordination and management of experimental, preclinical, and clinical studies, and the planning and submission of regulatory filings.

Charles Saltzman, MD.

Dr. Saltzman is the Louis S Peery Endowed Presidential Professor and Chairman of the Department of Orthopaedics at the University Orthopaedic Center of the University of Utah, and specializes in surgery of the foot and ankle. He received his undergraduate degree at Brown University, and medical degree from the University Of North Carolina School Of Medicine. He has authored over 130 peer-reviewed publications, 30 book chapters, and co-edited three major textbooks including Surgery of the Foot and Ankle. He is the immediate Past President of the American Orthopaedic Foot and Ankle Society.
